Hiho, here are two tools:

 

WWPFUEXT  - WWPack v1.12� Fixup Extractor, after unpacking with PROCDUMP, this program reconstructs the FIXUP Table

LINF2PEF - If you have a linear list of sorted Fixup adresses, it converts them into a FIXUP Table in PE format
